ASSESSMENT OF THE ORGANIZATION OF MEDICAL CARE FOR ADULTS WITH CORONARY HEART DISEASE IN A MEDICAL ORGANIZATION PROVIDING PRIMARY HEALTH CARE

Summary:
Introduction. Coronary heart disease continues to be one of the leading causes of death and disability of the adult population in the Russian Federation, which defines it not only as a medical problem, but also as an important socio-economic problem. According to official statistics, in recent years, the general and primary incidence of coronary heart disease in the country has shown a steady upward trend, therefore, an assessment of the organization of modern approaches to medical care will not only identify problems, but also identify potential areas for their optimization, ways and directions of their correction.
The purpose of the study. To evaluate the organization of medical care for adults with coronary heart disease in a medical organization providing primary health care in the Moscow Region (using the example of the Kotelniki urban district) for the period from 2019 to 2023.
Materials and methods. The official information of Rosstat published in the open press - the number and structure of the adult population of the Moscow region; data from "Monitoring the effectiveness of measures to reduce mortality in medical organizations providing primary health care provided by district therapists (paramedics) and general practitioners serving the population of the Russian Federation according to the territorial-district principle" have been studied. Moscow region; statistical forms of the Federal Statistical Office №№12 and 30; analytical materials of the annual reports of the medical organization for 2019-2023.
Results. The improvement of the diagnostic and logistical base of the polyclinic was noted in the Kotelnikovskaya Polyclinic during the study period. Outpatient medical care is provided using the multidisciplinary principle of managing a patient with coronary heart disease, led by a cardiologist. By 2023, the number of emergency medical care calls to patients undergoing outpatient follow–up with a diagnosis of coronary heart disease reached a minimum of 0 calls. In the structure of patients over 65 years of age who are under medical supervision in 2023 There was an increase of 5.98% in the observed men, which indicates the effectiveness of the measures taken.
Discussion. Competent organization of medical care for adults with coronary heart disease in a medical organization providing primary health care, preventive measures, dynamic follow-up, and the use of multidisciplinary patient management play a key role in reducing morbidity, preventing complications, and mortality.
Conclusion. The conducted assessment of the organization of medical care for the adult population indicates that with the constant growth dynamics of the attached population, including in the age group over 65, there is a stable staff and an improvement in the material and technical base at the Kotelnikovskaya Polyclinic. The multidisciplinary principle of managing a patient with coronary heart disease, timely identification of risk factors for chronic noncommunicable diseases and their accounting, continuity of care in outpatient and inpatient settings, an established routing control system, and follow-up lead to a reduction in the risk of complications and improve long-term prognoses.
