THE SPECTRUM OF DISEASES OF THE GASTROINTESTINAL TRACT AT AN OUTPATIENT APPOINTMENT WITH A GASTROENTEROLOGIST AT THE CHILDREN'S POLYCLINIC

Summary:
Annotation. Diseases of the gastrointestinal tract in children are an important socio-economic problem due to difficulties in diagnosis, treatment and possible complications. One of the main problems for many decades has been the diagnosis and treatment of chronic gastritis and gastroduodenitis, both in outpatient and inpatient settings. In recent years, there has been a rejuvenation of gastroenterological diseases and an increase in their number, especially in older school-age children. Implementation of the regulations for the treatment of gastritis and gastroduodenitis in accordance with the Maachstrich Consensus V (2017 ) and VI (2022) cycles of triple and quadrotherapy are carried out both for adequate treatment and for the prevention of complications. Goal. To determine the spectrum, trigger factors, and clinical manifestations of the main diseases of the gastrointestinal tract (GIT) recorded at an outpatient appointment with a gastroenterologist, as well as the effectiveness of diagnosis, therapy, and recurrence rates in patients with gastritis and gastroduodenitis. Materials and methods. 200 children with various gastrointestinal pathologies aged from 5 to 14 years were examined. The analysis of the distribution of patients by nosological forms was carried out with the definition of clinical features, concomitant diseases, trigger factors, the diagnostic detectability of Helicobacter pylori (H.P.) and the effectiveness of eradication therapy were determined. All children underwent EGDS, ultrasound of internal organs, biochemistry with determination of liver and pancreatic function, determination of HP using both invasive and non-invasive methods. In case of intestinal motility disorders, a study was conducted on microbial contamination and excessive bacterial growth of the microflora. The effectiveness of primary and repeated eradication courses for chronic gastritis and gastroduodenitis, as well as relapses at various times after the end of eradication therapy, was evaluated. The level of statistical significance of the differences was determined using Pearson's Chi-square test. The differences were considered significant at ptrigger factors, the determining influence is attributed to malnutrition and stress, and the associated diseases are mainly represented by obesity and impaired colonization of the intestine. Functional diseases of the gastrointestinal tract were mainly found in children of primary school age. The variety of noninvasive methods used in gastroenterology for determining HP does not differ significantly. There was no recurrence of the disease in all patients with gastritis who initially received three-component eradication therapy. The treatment strategy using three-component eradication N.R. has clear advantages over therapy with other groups of drugs, as it provides long-term remission and subsequent recovery. Relapses of the disease occurred with dieting disorders, intense physical exertion, the resumption of smoking in adolescents, the continuing stressful situation at school and at home, and an increase in body weight of more than 10%. Individual patients required routine hospitalization in a specialized department.
