OUTCOME OF THE PROSTATE CANCER EARLY DIAGNOSIS PROGRAM INVOLVING PUBLIC AND PRIVATE HEALTH SYSTEMS INSTITUTIONS

Summary:
Relevance. Prostate cancer remains one of the most significant oncological problems: 1.46 million new cases were registered globally in 2022, and the incidence rate continues to rise in Russia. In this context, early diagnosis and shortening the time to treatment are particularly important. Public-private partnerships, which provide more flexible patient routing and increased access to specialized care, can provide additional organizational resources.
Objective: To evaluate the effectiveness of a prostate cancer early diagnosis and treatment program implemented through a public-private partnership.
Materials and Methods: The study was conducted at City Hospital No. 115 and Clinical Hospital No. 1 "Medsi" from January 2019 to May 2020. As part of an organizational experiment, patients without a referral underwent PSA testing. If the test was elevated, they were referred via a call center to a urologist for further evaluation, including transrectal ultrasound, digital prostate examination, and prostate biopsy. Preoperative examination and hospitalization were organized within a short timeframe. Patients were referred for treatment at "Medsi" Clinical Hospital No. 1 under the public-private partnership program.
Results. Of the 585 program participants, indications for biopsy were identified in 162 patients, with an average age of 65.1 ± 7.2 years. A total of 166 biopsies were performed, with prostate cancer histologically confirmed in 61 patients, representing 37.7% of those examined. 159 biopsies (95.8%) were performed under ultrasound guidance. No more than 14 days elapsed from receiving the PSA result to confirming the diagnosis. Thirty-two patients completed the full diagnostic and treatment cycle at "Medsi" Clinical Hospital No. 1.
Discussion. The data obtained indicate that the public-private partnership model reduces diagnostic timelines and ensures continuity between outpatient and inpatient care. A significant result was the high early-stage cancer detection rate among surgically treated patients.
Conclusion. The implemented program demonstrated organizational and clinical effectiveness: it ensured timely diagnosis, adherence to treatment schedules, and the possibility of a full treatment cycle under the compulsory medical insurance system. This model can be used to expand access to oncology care.
