DEVELOPMENT OF A MODERN MODEL FOR ORGANIZING MEDICAL CARE FOR PATIENTS WITH MALIGNANT BLOOD CANCERS IN THE SPECIALTY OF «ONCOLOGY»

Summary:
Significance. Advances in the treatment of malignant tumors sometimes prevent patients from achieving a long, active life and significantly improving their quality of life due to the negative impact of cancer medications and the side effects of radiation therapy. The development of a new model for organizing medical care for patients with malignant tumors, including timely diagnosis, prevention, and treatment of cardiovascular toxicity from anticancer therapy, will help address the problem of severe and life-threatening cardiovascular complications in patients with malignant tumors, including those affecting the hematopoietic system. The purpose of the study development and scientific substantiation of measures to improve the organization of medical care for patients with malignant neoplasms of the blood system in the oncology profile. Material and methods: A comparative analysis of current procedures and standards for providing medical care to patients with malignant neoplasms of the blood system, regional regulations in the Moscow region, and international clinical guidelines revealed significant differences in approaches to organizing medical care for such patients. This formed the basis for the development of a new model for organizing medical care for patients with malignant neoplasms of the blood system in the oncology specialty. Results. A new model for organizing medical care for patients with malignant neoplasms of the hematopoietic system has been developed at the Oncohematology Department of the Lapino Central Clinical Hospital, which opened in 2020. The key principle of this model is interdisciplinary patient management, timely prevention, diagnosis, and treatment of cardiovascular complications to improve the quality of life and prognosis of patients. Conclusion. Regulatory inclusion of cardiologists in the provision of medical care to patients with malignant neoplasms, as well as the development of clinical guidelines containing an interdisciplinary approach to the organization of treatment of patients before cardiovascular toxic therapy, will significantly improve the safety and quality of medical care for patients with malignant neoplasms of the blood system.
