ANALYSIS OF STAFFING OF THERAPISTS AND GENERAL PRACTITIONERS IN MEDICAL ORGANIZATIONS IN THE FAR EASTERN FEDERAL DISTRICT

Summary:
Primary health care is a critical component of the Russian Federation's healthcare system, determining the accessibility and quality of medical services for the population. The Far Eastern Federal District, which occupies 40.6% of the country's area and is characterized by exceptional territorial and demographic complexity, faces an acute shortage of medical personnel. The period 2019–2024 was marked by large-scale transformations in the regional healthcare system's personnel structure, requiring a comprehensive analysis of the alignment of the number of physicians with the actual needs of the population. Purpose: A comprehensive analysis of the dynamics of staffing indicators for general practitioners and primary care physicians in the Far Eastern Federal District. Materials and methods: A study of staffing indicators for primary care physicians and general practitioners from 2019–2024. Source of information: Federal Statistical Observation Form No. 30 "Information on a Medical Organization." We calculated staffing indicators for primary care healthcare organizations: the staffing levels of full-time general practitioners and therapists, the population density of general practitioners and therapists (per 10,000 adults), and the part-time employment rate in the Far Eastern Federal District over the period 2019–2024. Research methods: analytical, statistical, and comparative analysis. Results. An analysis of staffing indicators for general practitioners revealed negative trends in the number of full-time positions, occupied positions, and the number of individuals. A different picture is observed for district general practitioners, reflecting contradictory but generally positive trends. A decrease in the number of full-time positions was observed, while the number of individuals increased and the number of occupied positions increased. Moreover, the rate of growth in the number of district general practitioners lags significantly behind the rate of decline in the number of full-time and occupied general practitioner positions, indicating a lack of HR initiatives. Conclusion: The study revealed the systemic priority of the district general practitioner model over the general practitioner model. The absolute shortage of physicians in both categories poses direct threats to the accessibility and quality of primary health care in the region. Overcoming this crisis requires a range of measures: intensifying recruitment efforts, optimizing working conditions, improving specialist training, and streamlining organizational processes for utilizing existing human resources.
