REMOTE MONITORING OF PATIENTS WITH INTRACARDIAC DEVICES - ADVANTAGES, OPPORTUNITIES AND PROSPECTS (REVIEW)

Summary:
Relevance. Implantation of intracardiac devices becomes more and more frequent method of arrhythmia and heart failure treatment. In Russia, pacemakers (ECS), cardioverter-defibrillators (ICD), resynchronizing devices are implanted annually in more than 300 patients per 1 million inhabitants of the country. The number of patients with these devices equipped with systems for remote monitoring of their operation and control of the cardiovascular system is increasing. Today remote monitoring optimizes doctors' work, saves patients' time, prevents decompensation of the latter's condition and even their mortality due to timely registration of life-threatening events and their urgent transfer to specialists in just a few minutes using the information network - “patient - service center - doctor” with the subsequent reaction of the latter. The article presents the results of studies confirming the effectiveness of these technologies, discusses the issues of improving the legal issues of telemedicine in cardiology and the confidentiality of the data obtained.
Research objective. To analyze the studies on the role of telemedicine tools in monitoring patients with implanted intracardiac devices, on the possibilities, problems and prospects of remote monitoring of the cardiovascular system of this category of patients.
Materials and methods. For the analysis we searched domestic and foreign papers in Elibrary, CyberLeninka, Pubmed, MEDLINE, Cochrane Library databases by keywords: “implantable intracardiac devices, arrhythmias, remote ECG monitoring, telemedicine. Most of the articles were dated 2014-2024 (last decade).
Results. The analysis of scientific articles by Russian and foreign authors devoted to the issues of application of one of the tools of modern telemedicine - remote monitoring of the cardiovascular system of patients with implanted intracardiac devices confirmed its relevance and undoubted usefulness for this cohort of patients and medical institutions serving them.
Conclusion. The continuing tendency to increase the life expectancy of patients with cardiovascular pathology and, consequently, the growth of their number actualizes the necessity to improve the methods of treatment of severe rhythm disturbances and severe heart failure. Among them, implantation of intracardiac devices is becoming more and more widespread. Many modern devices have capabilities of long-term remote control of their own functions and registration of arrhythmia episodes in patients, which allows to protect the latter from acute cardiovascular events even at a sufficient distance from specialized medical centers, as well as to optimize the work of the doctor, eliminating time spent on consultative appointments without indications. Every year, scientific studies confirming the versatile positive possibilities of these technologies appear, some of which the authors of this paper tried to summarize.
