Introduction. Maternal near miss requires urgent resuscitation and intensive care during pregnancy and postpartum period up to 42 days, creating significant difficulties in management and requiring interdisciplinary approach and modern treatment methods. Aim of the study. To investigate the features of organization and functioning of remote obstetric consultation centers (ROCC) in Moscow and St. Petersburg cities, analyze directions of their activities and assess the efficiency of operation. Materials and research methods. The study is based on studying current regulatory legal acts establishing ROCC operating procedure, ROCC performance reports. Comparative and analytical approaches were used, allowing to identify typical features and peculiarities of ROCC organization in each region in the period from 2021 to 2025. Research results and discussion. ROCC in Moscow and St. Petersburg operate 24/7/365, organizing consultations using telemedicine technologies. In St. Petersburg, ROCC was organized in 2019, there is division of center into planned and emergency departments performing according to federal order additional functions due to increase in staff number, as well as presence of mobile obstetric anesthesiology-resuscitation brigade in composition. In Moscow city ROCC was created in February 2021. Features of functioning are: conducting consultations in “doctor-patient” mode; absence of anesthesiology-resuscitation obstetric mobile teams in center composition. Conclusions. In St. Petersburg and Moscow despite regulation by single federal order, a number of similarities and differences in functioning and organization of ROCC were revealed, while activity of both centers is aimed at improvement of quality of medical aid provision, increasing efficiency of diagnostics, successful treatment of maternal near miss.
