INFLAMMAGING IN DEVELOPMENT OF PROXY DISEASES OF AGING

Summary:
Introduction. Chronic low-grade systemic inflammation (inflammation) is a key mechanism in the transition from physiological aging to age-associated pathology and accelerated aging. Under the influence of intrinsic factors (SASP, DAMP) leading to the accumulation of senescent cells and extrinsic factors such as oxidative stress, obesity, and exotoxins, inflammation triggers the uncontrolled production of proinflammatory cytokines (IL-6, TNF-α, IL-1β), creating a vicious cycle of damage and accelerated aging proxy diseases of aging. Clinically, this manifests as chronic fatigue, cognitive impairment, early dermatological changes, muscle and joint pain, and elevated systemic inflammatory markers (C-reactive protein). Understanding the molecular mechanisms, comprehensive diagnostics of inflammatory markers, and timely lifestyle modification are considered strategic directions for increasing healthy lifespan. This study is devoted to the analysis of key links in inflammaging and the substantiation of approaches to its early diagnosis and prevention. Objective. To study the clinical manifestations of inflammaging in patients (30-65 years) with early signs of inflammatory aging. Materials and methods. The study included 126 patients aged 30-65 years, average age 48.5±8.3 years, who sought medical attention at a clinic with signs of early aging, including metabolic risks, overweight or obesity, hypertension and subjective symptoms. All patients underwent a questionnaire, laboratory diagnostics to determine the level of glycemia, lipid spectrum, determination of high-sensitivity C-reactive protein (hsCRP), levels of pro-inflammatory cytokines (IL-6, TNF-α). To assess the signs of systemic inflammation, a main group of 47 people with an elevated high-sensitivity C-reactive protein (hsCRP>3 mg/l) and a control group of 49 people were identified. Results and discussion. The study revealed that 78% of patients complained of chronic fatigue and low mood, while 65% reported pain in the lumbar spine and lower extremity joints. Most patients (75%) showed skin changes, including early wrinkles, decreased elasticity, and dry skin. Subcutaneous fat changes, such as facial atrophy, were detected in 30% of patients, increased abdominal fat in 45% of patients, and elevated C-reactive protein (>5 mg/L) in 85% of patients. The study compared clinical and laboratory parameters in patients based on their high-sensitivity C-reactive protein (hsCRP) levels.
