PSORIATIC ARTHRITIS AND DISABILITY IN PSORIASIS: CLINICAL AND SOCIAL PREDICTORS OF LOSS OF ABILITY TO WORK

Summary:
Introduction. Psoriatic arthritis is a key factor in persistent functional limitations and disability, often necessitating referral for medical and social disability assessment. Objective: to assess the contribution of psoriatic arthritis to the risk of disability in patients with psoriasis, and to compare the dynamics of social and labor outcomes in HIBT and traditional systemic therapy. Materials and Methods. A secondary analysis of a dissertation sample was conducted: 300 adult patients with psoriasis (18–75 years old) in three groups of 100 observations each (GIBT; traditional systemic therapy; retrospective control). The presence of PsA, severity indicators (PASI, BSA), quality of life (DLQI), and social and occupational outcomes were assessed. Results. PsA was detected in 12% of patients. At baseline, decreased professional activity was noted in 38%, and referral to the medical and social expertise was reported in approximately 22%. After 12 months, the frequency of referrals to the medical and social expertise decreased from 24% to 6% in the GIBT group and from 21% to 15% in the traditional therapy group. In the control group, there were no significant changes (20% and 18%, respectively). In the GIBT group, the number of days of temporary disability decreased more than 50% (24.6 - 10.8 days/year), and work limitations decreased from 40% to 11%; with traditional therapy, improvements were moderate. Conclusion. The most significant predictors of disability in psoriasis are the presence of PsA, severe/widespread course of the disease, a significant decrease in quality of life, and social maladjustment. Achieving deep disease control, especially in the context of GIBT, is associated with a reduced need for medical and social expertise and a reduction in work losses.
