DIGITAL ROUTING OF DENTAL PATIENTS WITH PRECANCEROUS DISEASES OF THE ORAL CAVITY: CHALLENGES AND OPPORTUNITIES

Summary:
Introduction. Digitalization of the healthcare system in the Russian Federation is recognized as one of the most important and modern tools for improving the quality and accessibility of medical care provided to the population, which opens up new opportunities for improving the routing of patients, including those with precancerous diseases of the oral cavity (PDOC), which are an important component, although not the most common reason for seeking dental care.
The purpose of the study is to increase the coverage and effectiveness of oncoscreening of primary dental patients through digitalization to differentiate and further route patients with PDOC.
Materials and methods. A retrospective analysis of the applies of primary patients to the Republican Dental Clinic of the Chuvash Republic Health Ministry in 2024 (RDC) for 2024, presented in the medical information system (MIS) "Statistics", was carried out. We also studied 10,504 questionnaires submitted to dental patients who applied to the RDC in 2024.
Results and discussion. In the structure of the primary dental morbidity of the Chuvash Republic population who applied to the RDC, diseases of the oral mucosa accounted for only 0.07%, which indicates a low incidence of patients seeking dental care for this pathology. The results of our study showed that in 2024, 10,504 (42.96%) completed questionnaires were submitted to doctors in conditions of fluidity and prevalence of inflammatory pathology. Of these, 72.31% of the c questionnaire were filled by the primary patients, while the complete and high-quality completion of the questionnaire for subsequent processing was no more than 52%. This leads to late apply to specialists and lost time for early diagnosis. Digital resources can help to raise awareness, but it is necessary to take into account the problem of information overload and the necessity to differentiate reliable sources. One of the most likely solutions is to integrate screening questions and information at the pre-registration stage on the portal. When making an appointment through Public Services, the patient is asked to fill out a short form that includes questions aimed to identifying risk factors for developing of the oral diseases. Based on the responses to the questionnaire and taking into account other data (age, gender), it is possible to assess the risk of developing PDOC at the stage of making an appointment with a specialist and, upon further treatment of such a patient at the dental clinic, to build routing in this clinic depending on the priority of dental pathology at the time of his attendance.
Conclusion. The advantages of implementing such a solution will be full coverage of primary patients who apply to medical organizations for dental care, increased detection of PDOC in the early stages and, as a result, improved prognosis and reduced mortality from oral cancer.
