CURRENT ASPECTS OF TEMPORARY DISABILITY EXAMINATION

Summary:
Introduction. The inclusion of the need to reduce the total duration of temporary disability among national healthcare goals has necessitated a detailed study of the organizational and legal aspects of temporary disability assessment. The objective of this study is to identify potential areas for improving the legal regulation of temporary disability assessments, aimed at eliminating unnecessary visits to healthcare facilities. Material and Methods. A content analysis was conducted of mandatory requirements for temporary disability assessments, as well as recommended forms of interaction between healthcare facilities and regulatory authorities. Results. Current legislation on temporary disability assessments separates the functions of the assessment itself and the recording of its results, allowing for the latter to be delegated to non-medical personnel. Furthermore, in cases of coronavirus infection, remote methods of closing a sick leave certificate are permitted. For a number of illnesses and injuries, the Russian Ministry of Health has already developed recommendations on the objective duration of disability, largely overriding the relevant decisions of the attending physician or medical commission. Moreover, the development of a digital society already envisages a transition to remote forms of interaction with healthcare facilities, for example, within the framework of state control (supervision). Discussion. High-quality medical care requires the rational use of available resources. The use of digital technologies allows, on the one hand, to increase potential population coverage and, on the other, to avoid excessive burden on healthcare workers and healthcare facilities' infrastructure, which ultimately improves access to medical care. Consequently, it is potentially advisable to consider granting constituent entities of the Russian Federation the authority to determine a list of subordinate medical organizations that can extend and close sick leave certificates remotely in the form of electronic documents, without an in-person appointment (examination, consultation), including through the use of telemedicine technologies, audio calls, or video calls. Conclusions. The conducted study suggests the potential to relieve physicians and medical commissions of some of the "formal work" associated with the in-person format of closing sick leave certificates. However, remote interaction formats, like any tool used in healthcare, must have strict indications and contraindications.
