PERSONNEL OF MEDICAL ORGANIZATIONS PROVIDING PRIMARY HEALTH CARE TO THE ADULT POPULATION: FOCUS ON PRIMARY CARE PHYSICIANS AND NURSES WORKING WITH THEM

Summary:
Introduction (relevance). The availability of sufficient qualified personnel is the first condition for ensuring the availability and quality of medical care. The article examines the trends in recent years in the number of employees of medical organizations involved in providing primary medical care to the adult population.
The aim is to assess the dynamics of the number of individuals of doctors involved in providing primary medical care to the adult population and nurses working with them over the period 2017-2024 in the Russian Federation, as well as to assess changes in the personnel structure of these groups of personnel.
Materials and methods: analysis of the number of individuals of doctors involved in providing primary medical care to the adult population and nurses working with them in 2017-2024 based on information from forms No. 30 of the federal statistical Office. The method of descriptive statistics is used.
Results. The indicators of the number of individual doctors (total), doctors working in units providing outpatient and inpatient medical care, primary care physicians and nurses working with them, as well as their ratios are analyzed. Main trends: an increase in the total number of doctors in 2024 mainly due to an increase in the number of doctors working in outpatient care units; an increase in the number of doctors providing primary medical care due to an increase in the number of district internists, a decrease in the number of nurses, more pronounced in terms of working with general practitioners. The differences between the subjects of the Russian Federation in the dynamics of the number of doctors providing primary medical care and nurses working with them are considered.
Discussion. The most stable trend in recent years has been an increase in the number of district internists. For a detailed analysis of the personnel situation, it is necessary to take into account the models of organization of the work of the district level.
Conclusions. The period 2017-2024 in the Russian Federation as a whole was characterized by an increase in the number of individuals providing primary medical care by 9% and a decrease in the number of district nurses by 8%. The shares of doctors providing primary medical care to the adult population and nurses working with them in the number of medical and nursing staff, respectively, were stable and had a positive increase over the study period. The number of individuals providing primary medical care to the adult population is dominated by district internists, their ratio increased to 4.75 per general practitioner in 2024. The ratio of nurses working with district internists and general practitioners also increased to 3.69:1. The analysis of the number of doctors providing primary medical care to the adult population and nurses working with them and the dynamics of the number showed multidirectional trends in the federal districts and subjects of the Russian Federation. For further analysis, it is necessary to use information about organizational decisions implemented in medical organizations of the subjects of the Russian Federation regarding the provision of primary medical care and related processes.
