INFLUENCE OF PLACE OF RESIDENCE ON THE CHARACTERISTICS OF HOSPITALIZATIONS OF CHILDREN WITH CEREBRAL PALSY

Summary:
Introduction. Maintaining and strengthening the health of the child population is the most important task of the healthcare system. In modern conditions, special attention is paid to improving the health indicators of children with disabilities and disabled children. Purpose: to assess the influence of place of residence on the characteristics of children with cerebral palsy hospitalized in a children's multidisciplinary hospital. Materials and methods: the data from the medical information system "Ariadna" on 568 children aged 0-17 years hospitalized with cerebral palsy in the psychoneurological department of the Children's Clinical Hospital of St. Petersburg State Pediatric Medical University in 2022-2024 were analyzed. For statistical processing and analysis of the obtained results, MS Office-2019 packages and the StatSoft-Statistica 10.0 statistical software package were used. Results and discussion: reliable regional differences in the age structure of patients with cerebral palsy were established (χ2=15.47; p=0.02), while they were absent in the gender structure (χ2=2.65; p=0.27). In the multidisciplinary children's hospital, planned hospitalization of primary pediatric patients prevails, carried out independently and paid for by high-tech medical care funds. No significant differences were found between the metropolis and the regions in the ratio of sources of payment for children's treatment (χ2=3.35; p=0.19), in the distribution of patients by form (χ2=1.25; p=0.54), repeat hospitalization (χ2=5.33 and 0.08) and the method of admission of patients with cerebral palsy to the hospital (χ2=0.75; p=0.69). The duration of hospitalization depends on the child’s place of residence (χ2=15.97; p=0.04), but the average duration of hospitalization is similar in all three groups of patients studied. Children from St. Petersburg are more often admitted to the hospital by referrals from city outpatient clinics, while regional patients are admitted mainly from children’s hospitals. The most common form of cerebral palsy, regardless of the region of residence, is spastic cerebral palsy (χ2=7.04; p=0.15). Almost all hospitalized children are classified as disabled children, the frequency of which does not differ between the city and the regions (χ2=0.86; p=0.65). The overwhelming majority of children are discharged with an improvement in their health, and no significant differences were found between the patient groups for these indicators (χ2=6.88 and 0.10). Conclusion: The influence of place of residence on the age and duration of hospitalization of pediatric patients with cerebral palsy hospitalized in a children's multidisciplinary hospital was established.
