EXPERT ASSESSMENT OF THE ORGANIZATION OF PROVIDING HIGH-TECH MEDICAL CARE

Summary:
Conduction. Currently, new approaches to the organization of the provision of the most expensive types of medical care continue to develop. The search for management solutions should be carried out taking into account regional peculiarities. An objective tool that allows us to talk about the presence of problematic areas in the organization of high-tech medical care and helps to identify the most significant of them is the expressed expert assessment.
Goal. To conduct an expert assessment of the organization of high-tech medical care at the level of the healthcare system of the constituent entity of the Russian Federation.
Materials and methods. A one-stage, prospective study (September 2024). To achieve this objective, the method of expert assessments was used, followed by the use of statistical and analytical methods. Experts (n=51), who are heads, deputies and heads of structural divisions of federal, state and private medical organizations, took part in the correspondence survey. To exclude the random nature of the identified problem areas, a study was conducted to determine the uniformity of the sample using a nonparametric the Cochran criterion (Q).
Results. In the course of the study, experts in the field of healthcare organization identified objective and reliable characteristics regarding organizational, resource and information problems of providing high-tech medical care. The leading problem identified by experts (35.3%) is the lack of a single registry for all medical organizations with the ability to ensure continuity of patient management before and after high-tech medical care. Low awareness of both medical staff (33.3%) and patients (21.6%) was noted. In addition, every second expert working in a state medical organization pointed to insufficient financial support and outdated logistical base. The obtained value of the Cochran criterion (Q) is more than 2.5 times higher than the tabular one, which with a high degree of reliability (p≤0.01) allowed us to conclude that the differences in the groups were not accidental.
Discussion. The conducted study made it possible to identify the most significant problem areas in the organization of high-tech medical care, which is confirmed by research by a number of authors in other regions of the Russian Federation.
Conclusion. The opinion of the expert community in the field of healthcare expressed in the course of the study is the basis for the search for new management solutions in the organization of high-tech medical care.
