QUALIFICATION STRUCTURE OF THE PARAMEDIC STAFF OF THE EMERGENCY MEDICAL SERVICE

Introduction. One of the directions of reforming the domestic healthcare system is the modernization of emergency medical care. The combination of the high social importance of this service with increased costs for its provision and the long-term staff shortage makes it necessary to seek new approaches to the organization of the pre-hospital health care, among these is the increased role of paramedical teams in emergency medical care. In this regard, an extremely urgent problem for modern healthcare is the assessment of the professional level of paramedic personnel of emergency medical teams in terms of readiness to assume new responsibilities in combination with an increase in their level of responsibility. The purpose of the study. To analyze and evaluate the qualification structure of the paramedic staff of emergency medical services in the Russian Federation. Materials and methods. The qualification structure of the paramedical staff of emergency medical care in the Russian Federation was investigated for 2018-2024. Materials of the annual reporting form of Federal statistical observation №30 «Information about a medical organization» for 2018-2024 were analyzed. Methods of research: analytical, descriptive statistics. Results and discussion. The staff of emergency medical care stations (departments) in the Russian Federation as a whole showed that the share of paramedics of field ambulance teams in 2024 amounted to 73% of the number of specialists with secondary medical education. In 2018-2024, the number of ambulance paramedics decreased by 6.5% across the country. The rate of provision of the population with paramedics of emergency medical care in 2024 nationwide amounted to 4.6 per 10,000 population. Over the period 2018-2024, there has been a downward trend in this indicator (from 4.8 to 4.6 per 10,000 population). In 2024, the staffing of paramedics in the country as a whole amounted to 86.3%. In 2018-2024, there is a downward trend in staffing levels from 89.8% in 2018 to 86.3% in 2024. The ratio of part-time paramedics of emergency medical services was 1.3. In 2024, the share of paramedics with the highest qualification category increased slightly compared to the previous year and amounted to 28.8%. Over 2018-2024, there has been a decrease in the proportion of paramedics with the highest qualification category (30.7% in 2018 to 28.8% in 2024), the first qualification category (from 13.7% in 2018 to 8.6% in 2024), the second qualification category (from 7.7% in 2018 to 4.2% in 2024). The analysis of the qualification structure of ambulance paramedics by subjects of the Russian Federation is characterized by significant differences between subjects, while a downward trend in qualification categories was noted in all federal districts. In addition to the existing shortage of personnel, which is noted by many researchers, the quality of medical personnel training, professional development and improvement of professional skills of mid-level specialists remains an urgent issue. The analysis of the qualification structure of paramedics in the Russian Federation showed significant regional imbalance in the availability of qualified categories of employees and a decrease in the proportion of people with the first and second categories in the Russian Federation. Conclusions. An analysis of the qualification structure of paramedics in emergency medical care revealed significant regional disparities in the availability of a qualification category among employees. A decrease in the proportion of persons with qualification categories was noted in all federal districts of the Russian Federation. The results of the study indicate a low interest of specialists in professional development, an attrition of employees and an aggravation of the personnel shortage.
