EVALUATION OF THE ACTIVITIES OF MOBILE MEDICAL TEAMS

Summary:
Introduction. The creation and development of a network of mobile medical teams facilitates increased access to medical care for the population, including in remote and hard-to-reach communities. Mobile medical organizations provide medical care to rural residents. The mobile nature of physicians and nursing staff has long been used as a model for delivering medical care. For example, in 1977, one of the areas for "improving public health" identified the urgent need to develop and organize mobile outpatient clinics, dental offices, and mobile medical teams. Experience has also demonstrated the effectiveness of these measures in many countries around the world, where the practice of mobile medical examinations and consultations with qualified specialists is actively developing. In modern conditions, the classification of mobile units of a medical organization includes more than ten broad categories, including one type—mobile medical teams. A mobile medical team is a structural subdivision of a medical organization that provides medical care to adults and children in populated areas, including those with poor transportation accessibility due to climatic and geographical conditions.
The purpose of the study. Conduct a retrospective analysis of the activities of mobile medical teams, identify the main criteria for their use, and systematize trips according to their purpose.
Materials and methods. The retrospective analysis was based on extracted summary data for the constituent entity of the Russian Federation from Federal Statistical Observation Form No. 30, "Information on a Medical Organization," from 2019 to 2024. The sample study was conducted at the Russian Research Institute of Health, based on data obtained through documented requests to executive authorities responsible for public health in the constituent entities of the Russian Federation. The request contained information on the activities of each mobile medical team, including the number of visits, settlements covered by the visits, travel time, and the number of patients admitted to a specific settlement by the end of 2024.
Results and Discussion. Over the past six years, the country's mobile medical team numbers have decreased slightly, but the number of patients seen during outings has increased, increasing the workload per team and the efficiency of their deployment. It has been established that the primary purpose of mobile medical team outings is to conduct preventive medical examinations and screenings for the adult population. Differences in the team's outing goals have been identified depending on the level and type of healthcare organization. It has been established that, on average, a single mobile medical team outing covers 3.2±0.67% of the population of a given community. The frequency of outings to the same community over the course of a year was 6.2±0.92. The average travel time to a community during a single outing was 57.8±1.88 minutes. Significant differences were revealed across the federal districts of the Russian Federation in the frequency of travel to one locality during the year (3.2-19.4 min) and the average travel time to a locality (31.7-112.9 min).
Conclusion. The identified differences in indicators characterizing the territorial characteristics of the constituent entities of the Russian Federation confirm the need for in-depth planning of the activities of outreach work of medical organizations, taking into account objective factors: service area, population density, transport accessibility, and travel time to the place of medical care.
