PATIENTS ASSESSMENT OF THE AVAILABILITY OF RADIATION DIAGNOSTICS IN THE FRAMEWORK OF PRIMARY SPECIALIZED ONCOLOGICAL HEALTH CARE

Summary:
Introduction. Improving the organization of medical care in oncology is one of the leading tasks of healthcare organizers. Radiation diagnostics should be performed in outpatient cancer care centers (OCCC) for early diagnosis and dynamic monitoring, and therefore its accessibility to patients is of great importance, which is assessed by interviewing them.
The purpose of the study: assessment by patients of the availability of radiation methods for patients in the diagnosis of malignant neoplasms in OCCC.
Materials and methods. The study was conducted using a sociological survey of 420 OCCC patients with different levels of medical equipment for radiation diagnostics using a specially developed questionnaire. The empirical data obtained were statistically processed with the calculation of extensive and intensive indicators, and the assessment of the reliability of their difference according to the Student's criterion.
Results. Radiography at diagnosis was performed in 36.6% of patients, mammography in 17.1%, ultrasound in 65.7%, in some cases, the studies were performed in excess of the standard waiting time. There are restrictions on the availability of CT and MRI when diagnosing cancer: in the CT-equipped OCCC, 20.1% of patients completed the study within the prescribed time frame and 15.3% of patients paid for the study; in the OCCC not equipped with CT, 1.2% met the waiting time standards and 35.6% of patients paid for the study. In the OCCC equipped with MRI, 5.0% of patients completed the study within the prescribed time frame, 17.9% of patients paid for the study; in the OCCC not equipped with MRI, 2.3% completed the study within the prescribed time frame, 40.1% of patients paid for the study. A similar trend towards limiting the availability of radiation diagnostics was established at the stage of clinical follow-up of OCCC patients. Satisfaction with the organization of radiation diagnostics in the more equipped OCCC 92.5%, in the less equipped OCCC – 35.3%.
Discussion. Limitations of the availability of radiation examinations for cancer patients are noted, which is reflected in the violation of the terms of provision and forced payment, which indicates that the OCCC does not fully fulfill the functions established for them and requires additional improvement in the provision of radiation diagnostics.
Conclusion. The main directions for improving the provision of radiation examinations are the creation of a patient routing system with the allocation of "quotas".
