CALCULATING EPIDEMIOLOGICAL INDICATORS OF TUBERCULOSIS AND MONITORING TUBERCULOSIS CARE: TIME FOR NEW APPROACHES

Summary:
Abstract. Changes in socio-economic conditions, the reform of the regulatory and legal framework for the provision of medical care - particularly phthisiatric care—to the population, the introduction of the national registry of tuberculosis patients, and new clinical guidelines have made it necessary to reconsider the approaches to the calculation and evaluation of several indicators reflecting tuberculosis epidemiology and the provision of phthisiatric care to the population. Methods. Based on the study of official statistical reports, the necessity for calculation or modification of methodologies for certain indicators was substantiated. Results. Changes in the procedure for dispensary observation and the reduction of treatment duration created a need to: 1) revise the source of information on tuberculosis incidence rate among the permanent population; 2) review the calculation of indicators for sputum conversion and clinical cure. Additionally, it became possible to monitor prevalently the ratio of patients receiving and not receiving treatment, as well as incurable patients. The implementation of molecular-genetic methods and the national registry of tuberculosis patients made it feasible and expedient to monitor indicators of coverage and quality of these methods’ application. Proposals for improving the monitoring of indicators reflecting the quality of active tuberculosis case detection have been logically justified. A methodology for calculating the following indicators was proposed and substantiated: tuberculosis incidence rate of the permanent population of a Russian Federation subject; proportions of patients detected actively, upon seeking medical care, and postmortem; coverage of pulmonary tuberculosis patients by molecular-genetic testing before the start of chemotherapy; confirmation of pulmonary tuberculosis using molecular-genetic methods; proportion of sputum-positive patients who ceased sputum positivity; proportion of clinically cured tuberculosis patients; tuberculosis patient mortality from all causes; mortality among sputum-positive patients; proportion of hospitalized tuberculosis patients, including sputum-positive ones. Conclusion. The modification of indicators will enhance their informativeness, enable objective reflection of the epidemiological situation in tuberculosis, and improve the assessment of the status and effectiveness of tuberculosis case detection, diagnosis, and treatment efforts, including by creating the possibility to apply standard biostatistical methods.
