ORGANIZATION OF MEDICAL CARE FOR THE ADULT POPULATION WITH ISCHEMIC HEART DISEASE IN THE AMUR REGION – A LOW POPULATION DENSITY REGION (2014–2023)

Summary:
Relevance. Ischemic heart disease is a widespread disease and the leading cause of death in the world and in Russia. In regions with low population density and large territorial extent, there are challenges that require special organizational approaches to providing cardiological care to patients with ischemic heart disease in order to maintain their health and prevent adverse outcomes. The Amur Region, with a population density of 2.1 people per square kilometer and a number of districts that are 600-900 kilometers away from the regional center, is a typical example of this Research objective: To evaluate the effectiveness of the existing system for organizing medical care for adults with coronary heart disease in the Amur Region and the measures taken to improve it. Materials and methods. To achieve this goal, we analyzed regulatory documents, performed a retrospective analysis of official statistical reports, and used quantitative methods, grouping, and comparing key indicators of the organization of cardiological medical care in the region for 2014-2023 for patients with coronary heart disease aged 18 years and older. The data was processed using Microsoft Excel 2010. Results and discussion. In 2023, compared to 2014, the level of provision of adult population with cardiologists increased with a continuing moderate shortage of individuals, there was an increase in the total (+15.9%) and primary (+30.6%) incidence of coronary heart disease, a decrease in the lethality and mortality rate from myocardial infarction. The time standards of indicators reflecting the growth of the availability and quality of emergency cardiological care for patients with acute coronary events were achieved, increasing their chances of a favorable outcome. The analysis of the use of cardiology beds indicates an intensification of the medical and diagnostic process. At the outpatient stage, the coverage of patients with coronary heart disease has been increased. The dynamics of the indicators confirm the growth of the efficiency of the organizational improvement of the process of providing cardiological care to residents of the region Thus, the creation of a target model for organizing specialized medical care for patients with coronary heart disease in the Amur Region allows for increasing its availability and quality, despite the epidemiological, demographic, and personnel challenges, as well as the territorial specifics of the region.
