Significance. In the demographic development system, mortality is a key factor determining the dynamics and structure of the population in different territories. Among the causes of death, violent mortality is one of the most controllable, which gives it a central place in public health strategies. The effectiveness of such strategies is evidenced by the significant reduction in road traffic accident mortality rates achieved through targeted programs at the federal and regional levels over the past decade. According to a global report by the World Health Organization, road traffic injuries remain one of the most serious public health problems worldwide. Despite the positive trends of recent years, the problem remains particularly acute in the Russian Federation. In accordance with the draft strategy for improving road safety in the Russian Federation, a strategic goal has been set for 2030, namely to reduce mortality on the country's roads by 1.5 times. The relevance of this goal is underscored by the scale of socio-economic losses, which, according to official data, exceeded 1 trillion rubles in 2023. Despite the positive trends noted by the ministry, the persistently high accident rate confirms the advisability of implementing a comprehensive strategic approach to solving this problem. Purpose. The purpose of this study is to examine the prevalence of road traffic injuries and fatalities in road traffic accidents using the Republic of Mari El as an example. Materials and Methods. The research was based on regional monitoring data for 2021-2024 and data from the Federal State Statistics Service of the Russian Federation (Rosstat). Descriptive statistics and comparative analysis methods were used in the study. Results. The article compares the main indicators of road traffic injuries for 2021-2024 using the example of the Republic of Mari El based on the results of regional monitoring. The study of the level and dynamics of injuries and mortality in road traffic accidents in the Republic of Mari El revealed that the development of trauma center infrastructure has not led to a stable increase in the effectiveness of medical care. Despite the creation of a three-level system and the growth of its material and technical potential, the key indicator – mortality among hospitalized patients – remains high, showing a negative trend in 2024. The main problem is concentrated in the pre-hospital stage and the first days of treatment, when more than 50% of the total number of deaths in hospitals occur, indicating insufficient effectiveness of emergency resuscitation care and routing. Conclusion. The developed set of measures, focused on optimising patient routing, strengthening the material and technical infrastructure, and implementing modern medical treatment approaches, represents a scientifically grounded tool for enhancing the accessibility and quality of medical care. Ultimately, this will help reduce mortality rates resulting from road traffic injuries in the region.
