THE RELATIONSHIP BETWEEN THE PSYCHOLOGICAL STATE OF DOCTORS AND THE FREQUENCY OF PATIENT COMPLAINTS

Summary:
It is generally accepted that psychosocial burnout is a systemic problem and does not belong to the area of responsibility of a single doctor. However, an analysis of patient complaints shows that quite a large part of the complaints is devoted to an insufficiently satisfactory attitude on the part of a medical professional. In general, the essence of the complaint can be viewed from two points of view: individual shortcomings of medical workers in terms of fulfilling their duties or shortcomings in the provision of medical services and problems at the organizational level. Can the personal characteristics of doctors contribute to increased conflict and provoke patients to write complaints?
Purpose. based on a survey of doctors, to determine the factors influencing their psychological perception of patient complaints and to analyze the relationship between these factors and the frequency of patient complaints about medical care.
Materials and methods. The appeals of citizens of the Moscow region to authorities of all levels accumulated in the ECSD of the Moscow region and attributed by artificial intelligence to the thematic category "Healthcare" for 2023 are analyzed. A survey was conducted of 2,862 doctors working in public medical organizations in the Moscow region.
Results. To clarify the clustering results, we identified five factors. The first factor is associated with the pleasure of doctors from their work. It builds resilience to emotional problems at work, is characterized by comfortable working conditions and psychological comfort in the team. The second factor reflects the increased anxiety of doctors. The third factor reflects the active lifestyle of doctors and their psychological stability. The fourth factor reflects the lack of mutual respect and cohesion in the team. The fifth factor is related to the psychological instability of doctors and their work experience.
Discussion. According to the results, two factors were identified that show doctors who do not have psychological and emotional difficulties, one factor reflects the effect on doctors of the team's lack of cohesion, and two factors are related to the poor / unstable psychological state of doctors. The second factor determined a certain wariness towards patients. These may indicate the anxiety of doctors. There are also doctors who seem to have quite an explosive personality. An interesting fact is that with these results, the proportion of unfounded complaints is lower, which may indicate that the explosive nature of doctors, their inability to cope with problems and, apparently, admit mistakes, forces management to agree with the complaints and admit that they are justified and the doctor really behaved incorrectly. Considering that these results speak about the personality traits of doctors, organizational and methodological measures at the level of the entire medical organization aimed at increasing the satisfaction of both the patient and the doctor will not be as effective as possible.
Conclusion. The unstable mental state of doctors is not related to the frequency of complaints from patients, but is related to the structure of complaints from patients. Anxiety and emotional difficulties of doctors affect not so much the relationship with the patient, but the quality of medical services provided.
