Summary:
Introduction. Diseases of the circulatory system, including those characterized by high blood pressure, continue to be the leading cause of death and disability worldwide. According to the Global Burden of Disease study for 2021, these conditions were responsible for 10.9 million deaths. Follow-up care, including the use of telemedicine technology, has proven to be effective, with 64.2% of patients achieving their blood pressure goals. A key role in this success is played by controlling modifiable behavioral risk factors with the guidance of doctors within a medical supervision framework.. In order to increase the effectiveness of secondary prevention measures for patients with high blood pressure, it is essential to evaluate the involvement of medical professionals in implementing strategies to address behavioral risk factors. Additionally, it is important to identify optimal methods for incorporating these approaches into clinical practice. The goal of this study is to identify and address problems related to the organization of outpatient follow-up for adults with high blood pressure. Furthermore, it aims to assess the current level of knowledge among medical professionals regarding preventive counseling and determine the need for additional training.
Materials and methods. The study was conducted as a one-time, anonymous survey. A total of 81 responses were collected. The questionnaire included questions about the participants' personal information, characteristics of the target population, assessment of the dispensary follow-up organization, self-reported knowledge of preventive counseling, use of information technology, and readiness for providing preventive counseling to patients with high blood pressure, including the use of telemedicine.
Results and discussion. Eighty-one Udmurt medical professionals participated in the study, mostly aged 20 to 40 years, with 61.7% being paramedics. Seventy-four percent of their patients were over the working age. The organization of dispensary supervision was rated 6 out of 10. Ninety-six percent of medical professionals provided preventive counseling, either verbally (47.5%) or in writing (29.6%). However, only 24.7% had completed training. To improve the organization of follow-up care, suggestions were made for training (34.6%), motivating patients (27.1%), providing additional time (24.7%) and improving technical equipment (13.6%).
Conclusions. As a result of the research, the main problems of dispensary supervision were identified, including the lack of staff, equipment, time for admission, and low patient motivation. It was found that only 24.7% of medical workers received training in preventive counseling as part of follow-up care for patients with high blood pressure. Additionally, the study revealed the willingness of medical professionals to use telemedicine technologies for preventive counseling.
