IMPROVEMENT OF ORGANIZATIONAL MEASURES FOR THE PREVENTION OF CEREBROVASCULAR DISEASES IN THE RUSSIAN FEDERATION: STATUS, PROBLEMS AND PROSPECTS

Summary:
Cerebrovascular diseases are the leading cause of death among adults, causing enormous economic losses to countries and society, and are a major medical and social problem worldwide. The problem of ischemic stroke prevention remains one of the most pressing today, as cerebrovascular accidents play a leading role among the causes of death and disability. The purpose of the study is a an analysis of the main risk factors for the development of cerebrovascular diseases using ischemic stroke as an example, identifying the prevalence of these factors among the population of the Russian Federation, as well as an understanding of current and promising methods that are used or can be applied by healthcare professionals to prevent the development of measures aimed at improving approaches to prevention. Materials and methods. Twenty-two published Russian and international scientific papers were analyzed. The review included materials related to the prevention of cerebrovascular and cardiovascular diseases in populations across various countries, as well as those describing new approaches to using digital tools for assessing the risk of developing these diseases. The results and their discussion. The growing acceptance and recognition of digital technologies for primary stroke prevention by laypeople, physicians, health policymakers, and regulators, coupled with their relative safety, accessibility (non-existent or low cost), and growing use worldwide, make them one of the most promising strategies for reducing the global stroke burden. Effective strategies for primary stroke prevention using various digital tools should be implemented alongside general population and other primary prevention strategies for cerebrovascular diseases in a combined and comprehensive manner. Conclusion. While significant advances in stroke treatment are evident, the prevalence of this condition remains high both in Russia and globally. The clinical application of scientifically proven methods for preventing stroke and vascular dementia has the potential to significantly reduce stroke incidence and disability associated with motor and cognitive impairment. There is growing evidence demonstrating the acceptability, feasibility, and effectiveness of various digital technologies for managing stroke risk factors and for the primary prevention of stroke and cardiovascular disease. When implementing preventive programs using digital risk assessment tools, it is essential to consider user motivation and long-term engagement, have a scientific evidence base supporting the use of digital risk assessment tools, consider potential challenges associated with the use of digital tools by the elderly and those from low socioeconomic groups, and address ethical and legal concerns regarding data privacy.
