STUDY OF THE OPINION OF MENTAL HEALTH PROFESSIONALS ABOUT CLINICAL RECOMMENDATIONS AND STANDARDS OF MEDICAL CARE IN PSYCHIATRY

Summary:
Introduction. According to Article 10 of the Federal Law of 21.11.2011 N 323-FZ “On the Fundamentals of Public Health Protection in the Russian Federation”, the availability and quality of medical care are ensured by the application of medical care procedures, clinical recommendations and standards of medical care. The latest amendments to the 323-FZ established that the transition of medical organizations to the provision of medical care on the basis of clinical recommendations will be fully implemented no later than January 1, 2025. Objective. To study the opinion of specialists in the field of mental health care about clinical recommendations and standards of medical care in the profile of “psychiatry”. Materials and methods. To conduct the study, we developed a special questionnaire that included 37 questions. The questionnaire was published on the Internet, and responses were collected from May 21 to June 3, 2024. Sociological and statistical methods of research were used. Results and their discussion. The questions of the anonymous questionnaire were answered by 536 specialists from the North-Western, North Caucasus, Southern and Volga Federal Districts. The results of the survey showed that the vast majority of specialists use CRs and standards in their work (97.2% and 94.6: respectively). The relevance, accessibility and practical usefulness of CRs and standards were highly appreciated by physicians. More than a quarter of the respondents indicated that they experienced some difficulties in using CRs (n=144; 27.6%), including: 1) lack of a number of drugs recommended in CRs (n=68; 13.1%); 2) lack of necessary material and technical base for implementation of CRs in the MO (n=49; 9.4%); 3) CRs are not always applicable in practice (n=28; 5.4%). The most common reasons for difficulties in working with the standards were: 1) Lack of medicines or lack of material and technical capacities specified in the standards (43.8%; n=35); 2) Practical inapplicability of the standards (15%; n=12); 3) Lack of specialists required to provide care in accordance with the standards (10%; n=8). Conclusions. The largest part of specialists positively evaluates the use of CRs and standards and is ready to use them in practice. At the same time, specialists note difficulties in using these documents. Specialists note that CRs excessively regulate their activities and do not allow taking into account individual characteristics of each patient. When implementing CRs, it is important to take into account organizational aspects, in particular, the MOs should have the required equipment, sufficient drug supply and the necessary staff.
