PHARMACOECONOMIC EVALUATION OF COSTS FOR CHRONIC PANCREATITIS THERAPY IN SPECIALIZED MEDICAL CARE

Summary:
Introduction. The incidence of chronic pancreatitis (CP) has more than doubled over the past 30 years. Effective therapy requires the use of pancreatin-based enzyme preparations in dosage forms (LF) that provide protection from the acidic environment of the stomach and have an optimal particle size. The development of methodological approaches to increase the availability of medicines for patients with CP based on a pharmacoeconomic assessment of the potential cost savings of compulsory medical insurance (CHI) in the procurement of innovative medicines is highly relevant.
Goal. Pharmacoeconomic assessment of the use of modified medicinal products containing pancreatin to determine the potential for saving direct medical costs for specialized medical care for patients with CP.
Materials and methods. The object of the study is the pharmacotherapy of patients with CP who were hospitalized in the therapeutic department of the Federal State Budgetary Institution KB No. 71 of the FMBA of Russia in Ozersk, Chelyabinsk region in 2023. The study carried out elements of pharmacoeconomic analysis based on data on the consumption of medicinal products with INN "Pancreatin". The cost-effectiveness analysis method was used to assess effectiveness, and the cost of 1,000 units of action was taken as a formal indicator of effectiveness.
Results. The results of a structural analysis of the drug supply from the anatomical-therapeutic-chemical group (ATC-group) "Polyenzyme preparations" were obtained, the qualitative characteristics of the studied group were studied, taking into account the specifics of the drug. The analysis of retail distribution in the regional pharmacy chain and on the main pharmaceutical marketplaces is carried out. Using standardizing documents, based on the analysis of primary data on the consumption of medicinal products in the study group, a cost calculation for CP pharmacotherapy in a hospital setting was obtained. A pharmacoeconomic assessment of the cost-effectiveness of the use of tableted and modified dosage forms of drugs containing pancreatin using the CEA analysis method was carried out.
Discussion. The predominance of tableted pancreatin drugs in the domestic pharmaceutical market has been determined. The proportion of tablet forms without modification of the release of the active ingredient is 66.7% in physical terms. The low share of CP pharmacotherapy costs in the provision of specialized medical care is shown - 0.06% of the total cost of hospitalization. An adapted CEA analysis of CP pharmacotherapy was conducted, based on alternative scenarios for building a procurement strategy for hospital drug provision, as a result, it was determined that the pharmacoeconomic effect of using modified forms corresponds to 74.34% savings in direct medical costs for pharmacotherapy.
Conclusion. The Russian pharmaceutical market is dominated by tablet forms of pancreatin, while modified forms included in the list of vital and essential medicines (VED) and recognized as the modern standard for enzyme replacement therapy for CP can significantly reduce the cost of pharmacotherapy by 74.34% while optimizing procurement strategies in medical organizations.
