Introduction. The article discusses the quality of medical services for women of fertile age. The performance indicators of the interdistrict perinatal center in the city of Achinsk of the Krasnoyarsk Region, as the only institution in the subject with the status of 2 and 3 levels of maternity care, and the only institution in the western district of the Krasnoyarsk Region to provide high-tech medical care in the "neonatology" profile, with services for women in labor and maternity hospitals of 11 municipalities of the region, were studied. Aim: to carry out an analytical collection of indicators characterizing the reproductive state of women of fertile age in a separate western district Krasnoyarsk Region to the constructed model of the organization of women's health protection on the basis of the perinatal center of the city of Achinsk. Material and methods. As a result of the study, a review of scientific and practical data was carried out, statistical information was studied using the licensed program IBM SPSS statistics 27.0.1 IF026 for 2019-2023. according to the region and medical organization, the author’s conclusions are drawn. Results. The analysis carried out in the city of Achinsk revealed that negative changes in demographic development have occurred within 5 years. There was a decrease in the number of births by 11.32% and an increase in the number of total deaths by 0.8%. In 2023, the average age of first childbirth for women of fertile age in Russia was 25-26 years, and according to the interdistrict perinatal center - 28-29 years. The Krasnoyarsk Region ranks fifth in terms of the highest number of abortions in the Russian Federation, more than 12 thousand abortions are registered every year. In 2023, the level of manifestation of pathologies of various types in women of fertile age increased in the Achinsk Perinatal Center. Inflammatory diseases of the female genital organs are the most common among women hospitalized in the gynecological department of the Achinsk Perinatal Center, the indicator of which increased from 49.2 to 53.6 (per 1000 female population) in the age category of 10+ years from 2019 to 2023.The problem of infertility also occupies a special place in the morbidity structure of women of fertile age. For 5 years in Achinsk, its dynamics has remained at the same level, without decreasing. An integral characteristic of women's reproductive health is maternal and perinatal mortality rates. In recent years, it has been possible to save all the women in the western district, which includes the Achinsk Perinatal Center as the main institution. Data for the subject as a whole and the Siberian Federal District remain higher for 2022 than for the Russian Federation. Perinatal mortality rates for the city of Achinsk are also better than data for the region as a whole and in the period 2019-2023, and the decline was 15.94%. Discussions. An analysis of the improvement of medical care, supplemented by medical documentation data, revealed effectiveness in indicators that directly involve the services of the only Perinatal center in the western district of the Krasnoyarsk Territory, but at the same time, most of the indicators influenced by factors at the stage of outpatient follow-up of women have decreased or remained at the same level for 5 years. It should be noted that the rapid rate of development of diagnostic databases in the Russian Federation is also affecting the increase in the incidence of women: the purchase of modern equipment and advanced training of specialist doctors. The role of changing the medical and social characteristics of women of reproductive age is not excluded. Conclusions. An analysis of the state of reproductive health of women in the western district of the Krasnoyarsk Territory revealed that there are problems in preserving the ecosystem for protecting the health of women of fertile age, and the features of the activities of the perinatal center for the prevention and prevention of gynecological diseases were highlighted. At this time, work with the health department in the region will lead to an increase in the level of reproductive health of women, an increase in the birth rate, and a decrease in maternal and infant mortality.
